What is ValleyConnect?
Powered by Unite Us, ValleyConnect is a regional Social Information Exchange Network connecting organizations across Mahoning and Trumbull counties. It creates a shared pathway for referrals, communication and follow-through — helping local organizations work together while continuing to do what each does best.

Talk to internal legal teams or leadership to determine if you are a Covered Entity and if a BAA is required. You will have the opportunity to note that upon submission of the Customer Registration Form (CRF), and a BAA will be sent to your organization for signature.

Why ValleyConnect?
Our region has many strong organizations and resources, but people can still experience disconnected systems, repeated intake processes, unclear referral pathways, and difficulty knowing whether a connection to support was completed. ValleyConnect helps strengthen the pathways between existing organizations. It is not a new service or a replacement for current programs. It is shared infrastructure that helps partners work together more effectively, improve coordination, and better understand community needs and service gaps.
How does ValleyConnect work?
ValleyConnect uses Unite Us, a secure platform that supports electronic, closed-loop referrals. With appropriate consent, participating organizations can connect individuals to services, communicate about referrals, and see whether a referral resulted in a connection to support. This creates a clearer pathway between organizations and helps reduce the need for individuals and families to navigate complex systems on their own.
Who can participate?
ValleyConnect is designed for Trumbull and Mahoning County organizations that play a role in supporting community wellbeing and opportunity. This may include nonprofit and community-based organizations, Federally Qualified Health Centers (FQHCs), hospitals and healthcare organizations, behavioral health providers, workforce and employment organizations, educational partners, public agencies, and other regional partners.

With appropriate consent, participating organizations can connect individuals to services, communicate about referrals, and see whether a referral resulted in a connection to support. How will ValleyConnect use data?
In addition to helping organizations coordinate individual referrals, ValleyConnect can provide a clearer picture of what is happening across our community. Aggregated network data can help partners better understand where needs are increasing, where referrals are being made, where gaps may exist, and how effectively people are being connected to support. The goal is to turn information into action, helping organizations, coalitions, funders, and community leaders make more informed decisions about services, partnerships, and investments. How are privacy and sensitive information protected? ValleyConnect is designed to support secure, consent-based information sharing. Participation does not mean that every organization can see everything about an individual. Access is based on appropriate permissions and the role of participating organizations, with information shared for authorized purposes.
What will success look like?
Success means more than the number of organizations using the platform. ValleyConnect is intended to strengthen how our regional system works together. Where will ValleyConnect serve?
ValleyConnect is being developed as a regional network serving Mahoning and Trumbull counties and bringing together partners across sectors to strengthen pathways to wellbeing and opportunity.
Is there a cost to participate?
Through regional funding secured for ValleyConnect, participation is free for organizations across the network. Non-clinical nonprofit organizations and Federally Qualified Health Centers (FQHCs) are eligible to participate in the Unite Us network at no cost. ValleyConnect funding also allows us to extend sponsored access to organizations that would otherwise require a paid license.
Hospitals may receive up to five sponsored licenses through ValleyConnect.
Our goal is to reduce cost as a barrier to participation so that organizations across sectors can be part of a stronger, more connected regional network.
When does ValleyConnect launch?
ValleyConnect will go live in October 27 2026. Organizations can begin learning about the network and preparing to participate ahead of launch.
